Community Health Needs Assessment
The Community Health Needs Assessment is an important initiative to identify key community needs and explore the ever changing health care landscape. It fulfills the requirements of a new federal statute established within the Patient Protection and Affordable Care Act (PPACA) requiring that nonprofit hospitals conduct CHNA's every three years.
Based on the findings of the Community Health Needs Assessment, each hospital develops an Implementation Strategy to address significant, identified health needs.
