Shontá Jones-Cole, DCM

Shontá Jones-Cole, LCSW, BCD, ACSW, CSWHC, CASWCM, is the director of case management. In this role, she oversees health care decision coordination and systematic approaches to treatment plans that improve quality and outcomes; coordination of care across the continuum and implementation of safe and appropriate discharge plans.

Previously, Shontá worked for The Department of Veterans Affairs and Los Angeles County, Department of Mental Health.

She earned bachelor’s degrees in psychology and child development from California State University of Northridge and a master’s degree in social work from the University of Southern California.

Shontá is a member of the National Association of Social Workers and the American Board of Clinical Social Work.
